Make a Personal Decision

It is important to have all your questions answered in order to make an informed decision. You are encouraged to call your local organ procurement organization for more information. If you are unsure that making an anatomical gift is the right decision for you, you are encouraged to speak to your doctor or clergy. Organ donation may not be the right option for everyone. The most important step in the process is telling your family. Someday, they may be in the position to fulfill your wishes.

If you decide to give an anatomical gift upon your death, you can indicate this by signing a donor card and informing your family. Some states have some way to indicate your intentions regarding organ donation on your driver’s license. Below is a sample donor card that you can print and sign. Be sure to have it witnessed. If you are under 18 years of age, have your parents witness it.
